QR code feedback forms customers can answer in seconds
Every Custovra form comes with a QR code and a short link. Print the code on tables, counters, receipts and packaging, and customers open your branded form straight from their phone camera.
What a QR code feedback form is
A QR code feedback form is a short online form that opens when someone scans a printed code with their phone. Modern iPhone and Android cameras read QR codes without a separate app, so the customer goes from scan to first question in a few seconds.
Compared with paper comment cards, nothing needs collecting or typing up, and every answer lands in your dashboard the moment it is submitted. Compared with emailed surveys, you ask while the experience is happening, which is when people remember it best.
How it works
- 01
Build the form
Choose a review or complaint form, then add star ratings, comments, multiple choice, staff and contact fields. Drag questions into the order you want.
- 02
Make it yours
Add your logo and set colours, fonts, button style and layout so the form matches your brand.
- 03
Get the QR code and link
Each form has its own QR code and link. Download the code for print, or share the link in messages and emails.
- 04
Watch responses arrive
New feedback appears in your dashboard in real time, with optional email alerts and reports per form.
What you can do with Custovra forms
Plan limits and which plans include each feature are on the pricing page.
Review and complaint forms
Two form types for two jobs: collecting ratings and praise, and handling problems.
Flexible questions
Star ratings, text, number, dropdown, radio, checkbox, staff selection, photo upload and contact fields.
Step-by-step mode
Show one question per screen with Back and Next buttons, which feels natural on a phone.
Embed on your website
Add the same form to your own site with a short embed snippet that sizes itself to the form.
Spam protection
Submissions pass a bot check, so your results reflect real customers.
Edit without reprinting
The QR code points to the form, not to a fixed copy of it, so you can change questions after printing.
QR Code Feedback Forms: frequently asked questions
Do customers need an app to scan the QR code?
No. The built-in camera on most modern iPhone and Android phones reads QR codes and opens the form in the browser. Customers do not need an account either.
Can I change my questions after printing the QR code?
Yes. The QR code links to your form, so edits to questions, colours or your logo show up without reprinting.
Can I use one QR code for several locations?
You can, but a separate form per location is better: each one gets its own QR code and results, and reports can compare locations. See pricing for how many forms each plan includes.
How big should I print the QR code?
A common rule of thumb is that a code can be scanned from about ten times its width, so a 3 cm code suits a table and a larger one suits a wall. Our step-by-step guide covers size, contrast and placement.
Can I put the form on my website as well?
Yes. Share the form link, or embed the form on your site with the embed snippet from your dashboard.
